Cadwell Dental Membership Details

Cadwell Dental Membership is an annual membership plan to make the new standard of modern dentistry you need more affordable.  We are committed to providing the highest quality dental services at Cadwell Dental at prices that make sense.  We offer the economies of group dental care with the uncompromised quality and attention of personal care.

Initial Examination: Just $59

• One time charge per person at joining
• Includes all necessary x-rays and thorough examination (78% off retail - normally $263)
• Fee is waived for current patients transferring to membership plan

Easy Monthly Dues

$19.50* per adult

$12.50* per child.  (First child is $19.50 if no adult members enrolled)

*EFT monthly rate with no handling fees.  $7 one-time enrollment fee per account.  Monthly handling fees will apply for monthly credit, debit, cash, and check payments.  Members may consider our no handling fee annual plans for all types of payment.  Call for further details.

Benefits 

• Your Cadwell Dental Membership provides you with the following:
• 2 hygiene visits (cleanings) per year (standard membership only, periodontal membership may be necessary for some individual health needs)
• 2 examinations per year
• All necessary x-rays
• 2 emergency visit exams per year during normal business hours (including prescription and necessary x-rays)
• In office fluoride treatments (1 per adult per year, 2 per child per year)
• Members save15-78% on additional services (services must be paid in full at time of service to receive member savings)

Upon enrollment, your benefits begin immediately. In order to take advantage of all the membership offers, members must enroll in 12 month increments. Enrollment after the 10th of the month will have the 12 month period begin on the 1st of the following month. For your convenience and to avoid uninterrupted care, your membership will automatically renew every year.

Periodontal Membership

3 hygiene visits per year - $34.50* per month

4 hygiene visits per year - $43.50* per month

• Includes all benefits listed above
• PLUS hygiene visits necessary to maintain a lifetime of healthy smiles
• Membership level necessary to maintain health is determined by the initial examination and subsequent yearly examinations.
• Periodontal infection control services may be necessary before entering into maintenance plan.  These services are eligible for 15% Member Savings and will be determined at initial examination.

Children

Children under 19 and full-time students up to 23 years of age are eligible for the child membership. Coverage of a child attaining the age of 19 will be continued if incapable of self-sustaining employment by reason of developmental disability or physical handicap.

Make Your Insurance Go Farther

We will continue to bill your dental insurance as a courtesy to you if you provide current information and updates when available. Your insurance checks can be sent directly to you to reimburse you for services provided. You may also elect to have your insurance reimbursement sent directly to Cadwell Dental. This will be applied to your Cadwell Dental Membership account, which can be used toward future services that may be necessary.

Roll-over Benefits Option

You may request a rebate at the end of the year for the amount of insurance and membership benefits paid but unused or you can save those benefits and allow them to roll into the next year. What a concept! It's great having the choice!
